You add pages and fields, set conditional rules and automations, publish a public link, and review submissions inside skillSYMS. This guide walks through building and publishing your first form.

Prerequisites

Before you begin, ensure you have:

  • An SDP or admin account with forms access
  • A clear idea of the data you need to collect
  • The fields and any required validation decided
  • A plan for where the public link will be shared

Step 1: Open the Forms Hub

Navigate to Forms β†’ Overview.

The hub lists your forms with their status (draft, published, archived) and submission counts. Use it to manage your form library.

Tip: Sketch your fields before you build. A clear structure up front saves rework once a form is published and collecting data.

Step 2: Create a Form

From the hub, click New Form, give it a name, and open the Builder.

Step 3: Add Pages and Fields

Navigate to Forms β†’ Builder.

  1. Add one or more pages to group related questions
  2. Add fields to each page (text, choice, date, file upload, etc.)
  3. Set labels, help text, and whether each field is required

Multi-page forms keep long forms manageable for respondents.

Step 4: Add Rules and Automations

Still in the builder:

  • Add rules to show or hide fields based on earlier answers
  • Add automations to trigger actions when a form is submitted

Rules keep forms relevant to each respondent; automations save manual follow-up.

Step 5: Publish the Form

From the builder or hub, click Publish. Publishing generates a public link you can share. You can unpublish or archive a form later without losing its submissions.

Navigate to Forms β†’ Public.

Copy the public form link and share it via Communications, email, or your website. Respondents complete the form without logging in, and file-upload fields accept attachments.

Step 7: Review Submissions

Navigate to Forms β†’ Submissions.

Every response appears here. Open a submission to view answers and attachments, and update its status as you process it.

Common Mistakes to Avoid

  1. Publishing before testing: Submit a test response before sharing widely.
  2. Too many fields: Ask only for what you need β€” long forms lose respondents.
  3. Forgetting required flags: Mark essential fields required to avoid incomplete data.
  4. Editing a live form carelessly: Changes affect new submissions; plan structural edits.

Verification Checklist

Before relying on the module, verify:

  • A form with at least one page and field exists
  • Required fields and any rules behave as expected
  • The form is published and a public link works
  • A test submission appears in Submissions
  • File uploads (if used) attach correctly
